Incident report form

The incident report form below will send a copy of your report to Cana's leaders.

A copy of the report will also be stored in a secure database so that Cana's leaders can retrieve a copy of the report at a future date if required.

The incident report form needs to be completed whenever:

  • A guest or volunteer is injured (even minor injuries.)
  • A guest has been asked to leave a shelter
  • Emergency services are called to a shelter

 

As well as submitting this form, please phone the leader of your shelter to make them aware that an incident has occured.
